So Im on the Xbox Beta app. If you clock your profile picture in the bottom right hand corner it will take you to your profile page. Once there just clock your profile picture. The first picture option will let you add custom picture.

Yeah the Xbox app doesn't seem to really work as it should, it is missing very main and basic feautres, but there is a workaround. I personally downloaded "Xbox Console Companion" onto my PC, and from there I do most of the Xbox profile management, such picture, clubs, activities etc. I hope that helps.
